For services available for adults who have a mental health diagnosis, such as:

Community Support Hours

Equip’s Community Support Hours service is an intensive and flexible mobile support service for adults aged 18 years and over who experience mental illness. The service is a wrap around one for individuals living in the community who have multiple needs. We support our clients spiritually, mentally and physically and offer a variety of services to suit their individual needs. Areas of assistance may include: housing work or study support to transition to different living arrangements widen friendship networks liaise with other services providing support to you involve family or friends in the wellbeing journey access to Equip’s own dietitian for assistance around weight management and eating well and affordably on a tight budget

Referral Process

You can access this service either via your GP, through the Co-ordination service of Te Whatu Ora Waitematā region or self referral by contacting us direct.

For over-65s, you can access this service via –

Te Whatu Ora - Health New Zealand Mental Health Services Older Adults (MHSOA) for the North Shore, West Auckland and Rodney Districts; or

Te Whatu Ora Koropiko Mental Health Services for Older People for Counties Manukau region.

Totara Club Dementia Day Care

The Tōtara Club is a community dementia day care service. It provides interactive day care for people diagnosed with mild to moderate dementia. Each club member has an individualised programme to meet their specific strengths and needs. The Tōtara Club also supports the family/whānau of people with dementia. The daily programme runs from 10.00am to 3.00pm Monday to Friday with lunch provided, and activities are designed to maintain and improve physical, social, cultural and cognitive well-being.

Description

The Tōtara Club is a community dementia day care service. It provides interactive day care for people diagnosed with mild to moderate dementia. Each club member has an individualised programme to meet their specific strengths and needs. The Tōtara Club also supports the family/whānau of people with dementia.

Koromiko House is a community respite service which provides 24 hour support in a high standard, homelike environment on the North Shore of Auckland. Access to the Respite Service is through direct referral from the Te Whatu Ora - Health New Zealand, Waitematā - Adult Mental Health Teams for North Shore (North 1 and 2), Waitakere and Rodney.
