Terryn Choat offers specialist nutrition assessment and advice for a wide range of dietary and health issues for older adults, living independently and in Aged Residential Care.
Expert dietetic input plays a vital role in reducing the risk of malnutrition, unintentional weight loss and promoting wound healing and managing chronic conditions (e.g. Chronic Kidney disease, Dementia, Parkinson's disease, Diabetes) safely and effectively within the community. A balanced, individualised diet helps seniors stay active, reduce hospital admissions, and maintain their independence for longer. Thoughtful nutritional care can also ease staff workloads and help reduce associated costs.
Appointments are available for anyone in New Zealand online or from two North Auckland locations.
For older adults I am able to:
- Nutritional assessments and tailored dietary interventions
- Monitoring and evaluating progress
- Implementing “Food First” strategies
- Prescribing Oral Nutrition Supplements
- Applying for special authority numbers
- Support to access meal and food services that meet individual food tastes, dietary needs and abilities
- IDDSI or support with texture modified foods
- Collaborating with chefs and kitchen staff
- Collaborating with care and nursing staff
- Review of tube feeding regimes (PEG or nasogastric) - Aged Residential Care, unless seen by DHB
For Aged Residential Care Facilities I am able to:
- Menu audits - these are required 2 yearly for aged care facilities, this is completed using the Dietitians NZ 'Food service and Nutrition audit tool'
- Provide support in the development of policy and systems for assessing, monitoring and managing malnutrition
- Providing support in the development of policy and guidelines for managing residents who have diabetes, are overweight or underweight or who need texture modified diets (from both a kitchen and ward perspective)
