Community Health Service

Today

Description

We work with older people and those living with a disability to help them live as well as possible in their own homes. Lifewise Community Support Workers help with a number of everyday tasks such as showering, meal preparation, laundry, shopping and medication prompts. Our support is designed to meet your needs and help you to still do what you can for yourself . This could mean we only come in once a fortnight or may involve helping you out several times a day.

A Lifewise Community Support Worker’s assistance could include helping you with a task, doing it for you, or assisting you to do it yourself. Our goal is to help you to be as independent as you can be.

We also support people who have chosen Individualised Funding (IF) as their disability support. IF can be simple and easy to use, and offers people more flexibility and choice about how their support works. We coach and guide people to make the best use of their funding, and help them to work out a budget. With IF you can employ your own Support Workers so that you have more control over your care.
