Description
Western BOP Primary Health Organisation is a health care organisation focused on providing care in the primary or community setting. Our Health and Wellness Service is Nurse-led and focused on providing community-based primary health care services.
We deliver a wide and expanding range of health and wellness services which include :
- Outreach and Community Clinics
- Support for Breast and Cervical Screening
- Self Management Groups
- Diabetes
- Weight Management
- General Long-term Conditions (LTC)
- Whānau Health Programme
- Workplace Wellness
We have a range of clinical staff including:
- Registered Nurses
- Dietitians
- Accredited Diabetes Nurse Specialist
- Nurse Practitioners
- Clinical Exercise Physiologist
Clinical Services include:
- Diabetes, heart and/or breathing conditions
- Hospital admission follow up
- ED referrals
- Cervical smears including transport and home visits
- Support to Breast Screening including transport
- Wound care
- Heart and BP checks
- Blood tests
- Smoking cessation
- Support with GP enrolment
- ACC
We also provide a Co-ordinated Primary Options Mental Health Programme or see here for all WBOPPHO Medical Centres.

Immunisation is the safest and most effective way to provide protection for you and your tamariki’s health. For more information view the NZ immunisation schedule. Outreach Immunisation Services (OIS): The PHO is sub-contracted to deliver Outreach Immunisation Services on behalf of its general practices, for children aged under 6 years, enrolled within WBOPPHO. These services focus on those children who have been identified as having missed some or all of their scheduled childhood immunisations and have not been immunised within the general practice they are enrolled with. Outreach Immunisation Services are one of an integrated set of strategies aimed at improving immunisation coverage and ensuring that people have access to services that empower them to make informed decisions regarding immunisation. The experienced Outreach vaccinators also endeavour to identify and vaccinate opportunistically (following informed consent), any whanau member present who may be eligible for approved funded vaccinations, in the various age groups.
