Health Coach
The Health Coach provides continuity of care along with emotional support and guidance. Coaches are integral members of the general practice team, assisting patients in navigating other services and gaining access to any social and cultural support they may need.

Health Improvement Practitioner
HIPs are qualified and experienced mental health professionals who have completed specialist training in the Te Tumu Waioira primary mental health model. The HIP is part of general practice team and sees people of ages and at all stages of their lives and with any type of issue related to behaviour change or mental wellbeing. The HIP will help the patient to agree on goals and put the necessary support, follow-up, and review in place to help patients achieve those goals.

Recommended after-hours telehealth provider: We recommend CareHQ as our preferred Telehealth GP service for patients, after-hours or when we are unavailable. The enrolled patient can access the CareHQ booking from the patient portal, MyIndici.
