About the Service
As part of Older Adults Service, the Assessment, Treatment & Rehabilitation (AT&R) Service provides a specialist inpatient and outpatient service dedicated to improving and maintaining the health and independence of older people. The service is a mix of geriatric medicine and rehabilitation for patients who often have multiple or complex needs, including those recovering from an injury or illness or who have an age related condition such as dementia or Parkinson disease.
The AT&R team help older adults manage the challenges associated with disability or aging that affect their day-to-day living. Working with the patient and their family/whānau, we plan their care and help the patient achieve their healthcare goals. The team may include doctors, nurses, physiotherapists, occupational therapists, speech language therapists, social workers, dietitians, pharmacists and needs assessors, depending on the patient's health needs and goals.
There are two inpatient wards at North Shore Hospital (Wards 14 and 15) and one at Waitakere Hospital (Muriwai Ward). We also provide an outpatient clinic and a domiciliary visiting service to patient’s homes and aged residential care facilities.
