Our Community Support service is an intensive and flexible mobile support service for adults aged 18 years and over who experience mental illness. The service is a wrap around one for individuals living in the community. Support Workers are client-centred, working alongside individuals to manage their own well-being. In addition to supporting individuals to manage their well-being, Community Support Workers can assist with the following: Widen social circles Find and maintain suitable housing Liaise with other services that are supporting you Involve whānau in your well-being journey Cultural support through Arataki’s kaumātua and kuia Spiritual support

Anyone aged 18 or over who has a mental health diagnosis is eligible for this service.
You can access this service through your community mental health nurse or by completing a referral form here. This service is provided in Whangārei and Kaipara.

In addition to supporting individuals to manage their well-being, Community Support Workers can assist with the following:
Support for mothers to recover from mental health difficulties between the first trimester of pregnancy through to their baby turning one year of age. Support you with your individual goals Provide mental health support and promote wellness Help you to access appropriate community resources or groups Offer support in caring for your baby as required Assist with pregnancy and birth planning support Liaise and work with your clinical team and other agencies you chose to be involved

We can recieve referrals from Northland DHB Maternal, Infant Mental Health team.
We also recieve referrals from midwives and GPs.

The service is provided in a home-like setting with three 2-bedroom flats. Staff are a mixture of registered nurses, social workers, and support workers.

Access to this service is through the Northern District Health Board, following an assessment by a service co-ordinator.

Health Improvement Practitioners (HIP) and Health Coaches (HC) work as part of the general practice team to provide individuals experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support. Arataki Ministries provides Health Coaches to General Practice Teams in Whangārei. The Health Coach can help patients with self-management support and with achieving their goals for improved mental wellbeing or lifestyle or behaviour change.
