Support you to access services relevant to your need. Work with you/ Whānau to plan an individualised flexible package of care to meet your identified need.

We take self referrals; you can make contact with us and we can complete a referral over the phone with you.
We take referrals from other agencies and you are most welcome to visit us regarding referrals.
