The service provides assessments and treatment for adults from 18 years plus suffering from a moderate to severe level of mental illness or addiction.
MidCentral Health Community Mental Health Services are provided by three locality teams based in Horowhenua, Palmerston North, and Tararua.
The Mental Health and Addiction Horowhenua Locality Service provides services covering Ōtaki, Foxton, Te Horo, Levin and Shannon.

What we do: The Horowhenua Community Mental Health & Addictions team provide: Triage and referral for the mental health & addiction needs for people experiencing moderate to severe mental health and/or, addictions concerns. Support to other service providers through collaboration and education. Assists with connection to support and advocacy services for people with mental health & addictions needs. Locality Based Delivery Model: UPC (Unplanned Care): provides short term mental health & addictions triage/ assessment, along with short term interventions for various mental health & addiction concerns, usually during and following a crisis. Horowhenua’s unplanned care services is delivered during daytime hours. Whaiora utilising this service will have an assessment upon referral and then be assisted to navigate their plan of care to the services that will best meet their needs as close to home as possible. This locality-based service will be provided through a roster of experienced mental health and addiction practitioners who work within the centre. This service provides a responsive navigation service for people who require unplanned care in the locality. FACT: The Care Coordination Team provides longer term treatment for people with complex mental health & addiction conditions; and provides care within a Flexible Assertive Community Treatment model (FACT). The person’s continuing and crisis treatment needs are provided by the one team. FACT aims to ensure continuity of care, to prevent admissions to hospitals and to foster inclusion, so that whaiora can participate in the community. Time Limited Therapies: The Care Coordination Team aims to provide short-term assessment and focused talking therapies. Time-Limited Therapies programme is an interdisciplinary team, and provides person and whānau specialist care for people who cannot have their mental health and addiction needs met in primary care. Interventions will include consultation, assessment, individualised and family based short-term treatment as well as therapeutic groups delivered in partnership with primary care and FACT services. The person’s continuing and crisis treatment needs are provided by one team. IDP: Our Interdisciplinary Care Coordination Team offers: Regularly reviewed intervention plans relating to mental health & addiction issues strengths assessments, and person-centred recovery or wellness plans. Whaanau Ora Kaitautoko: Our whaanau Ora kaimahi offer support to both whaiora & whaanau, supporting to achieving dreams, aspirations & goals that is whaanau whaiora led.
