Description

What the service is 

The Manawatu Community Mental Health & Addictions team works with Whaiora (people needing services) and their Whaanau (family), general practitioners/nurse practitioners and other treatment providers to help provide care and promote recovery for adults needing specialist mental health and addiction treatment in our community. 


What we do
 

The Manawatu Community Mental Health & Addictions team provide: 

  • Advice, assessment, treatment and therapies for people who require specialist mental health and addiction care 

  • Support to other service providers through collaboration and education. 

  • Assists with connection to support and advocacy services for people with mental health & addictions needs.


Locality Based Delivery Model
 

UPC (Unplanned Care): provides urgent mental health & addictions triage/ assessment, along with crisis interventions for people who are acutely mentally unwell. Manawatu unplanned care services is delivered during daytime hours. Whaiora utilising this service will have an assessment upon referral and then be assisted to navigate their plan of care to the services that will best meet their needs as close to home as possible. This locality-based service will be provided through a roster of experienced mental health and addiction practitioners who work within the centre. This service provides a responsive navigation service for people who require unplanned care in the locality. 

FACT: The Care Coordination Team provides longer term treatment for people with complex mental health & addiction conditions; and provides care within a Flexible Assertive Community Treatment model (FACT). The person’s needs are provided by the one team. FACT aims to ensure continuity of care, to prevent admissions to hospitals and to foster inclusion, so that whaiora can maximise independence and be connected to community and whānau supports.  

Time Limited Therapies: This specialist service is short term and provides assessment, treatment and talking therapies for people who cannot have their mental health needs met within primary care.  An interdisciplinary team works together with the Whaiora and Whānau on a plan to address short term needs. Interventions will include consultation, assessment, individualised and family based short-term treatment as well as therapeutic groups delivered in partnership with primary care.  

IDP: Our Interdisciplinary Care Coordination Team offers: 

  • Regularly reviewed intervention plans relating to mental health & addiction issues, strengths assessments and person-centred recovery or wellness plans. 

Whaanau Ora Kaitautoko: Our whaanau Ora kaimahi offer support to both whaiora & whaanau, supporting to achieving dreams, aspirations & goals that is whaanau whaiora led. 

The service operates from Palmerston North Hospital, Ruahine street. Clinics are held in Feilding

This service provides culturally appropriate assessments and treatment for adults from 18 years old living with a moderate to severe level of mental illness residing in the Manawatu region. Services include supportive care/treatment during acute phases of unwellness, medication prescribing and monitoring, individual therapy e.g. DBT (Dialect Behaviour Therapy) , ACT (Acceptance and Committment Therapy) CBT (Cognitive Behaviour Therapy), relapse prevention planning and recovery focused work with individual clinicians.

Referral Process

Referrals are accepted via self-referral, GP or community agencies for individuals, aged 18 years plus, in Palmerston North.

Existing service users are encouraged to contact their care coordinator in the first instance.

Eating Disorder Services

Specialty Eating Disorder care is delivered through the Adult Community Mental Health Team for clients 18 years and over. Following assessment, clients are assigned a care coordinator, with psychiatry, medical, dietitian and psychology input being individualised to their treatment plan.
