Mental Health & Addiction Support Services Co-ordination provides an overview of available supports which enable consumers of Mental Health and Addiction secondary services to actively participate in their recovery. Support Services Co-ordination have an active role in service planning and coordination, and liaison support with local, regional and national service providers. A Wellness Plan is a tool to help find out what effect a mental illness and/or addiction has on a person's life. It helps identify the abilities, resources, goals, needs and the supports which would be useful to themselves and their family and whānau. Wellness Plans are completed to put appropriate supports in place. Copies are usually held by those people and agencies who support you. We ask you to approve the release of this information to these people/agencies. If they do not have this information, they may not be able to properly support you.
