Description

Social Work Service

Social Workers are registered health professionals who work together with health practitioners to provide a comprehensive service to patients and their family/whaanau.

Social Workers undertake a comprehensive biopsychosocial assessment with patients.  This includes a review of a patient's social, emotional, spiritual, environmental, financial and support needs and determines how this may impact on the patient's health and wellbeing. CM Health Social Workers also support the patient and their family/whaanau with accessing supports. Social Workers work in the hospital as well as with outpatients and community. 

Social workers work across a range of areas such as: 

  • Medical wards
  • Surgical wards
  • Women's Health
  • Maternity
  • Paediatrics
  • Renal Medicine
  • Emergency Department
  • Cancer Psychological Support
  • National Burns Centre
  • Assessment Treatment and Rehabilitation
  • Child Development Community Services
  • Centre for Youth Health
  • Community Health teams
  • Auckland Spinal Rehabilitation Unit
  • Māori Health Te kahui Ora
  • Pacifica Health Fanau Ola

Referral Expectations

A referral to see a social worker can be made by any other health professional or patients can self refer.

Referral Criteria to Social Work Service:

  • Supportive counselling – grief and loss, impact of hospitalisation, lifestyle changes, adjustment to newly diagnosed health status and traumatic births
  • Care and protection of children 
  • Discharge planning
  • Trauma - accident, amputations, acute presentation of illness
  • Stress and anxiety
  • Family violence/Intimate Partner Violence
  • Elder abuse and neglect
  • Suicide
  • Terminal illness - palliative care
  • Death
  • Drug and alcohol
  • Conflict resolution - problem solving
  • Family/Carer stress
  • Frequent admissions

Fees and Charges Description

There are no charges for services to public patients if you are lawfully in New Zealand and meet the eligibility criteria set by the Ministry of Health.

If you do not meet the criteria, you will be required to pay for the full costs of any social work service that you need.
