Our Needs Assessment Service staff assess and coordinate supports for people who struggle with daily activities because of a health condition or disability.
The needs assessment service comprises two teams:
- Health of Older People - Over 65 years (HOP)
- Disability (Support Works)
Health of Older People - We develop a meaningful plan to help people stay safe and independently living in their home. We can refer to other services and agencies and link people with community support providers. We also complete assessments that inform the decision to move into Aged Residential Care. Another funding stream is supporting children and younger adults living with a Long Term Chronic Health Condition.
Disability - Support services are available to people with a physical, intellectual or sensory disability, including Autism Spectrum Disorder under the age of 65 years which is likely to continue to six months and requires ongoing support to live independently. We work alongside the disabled person to develop a plan that supports them to build a good life in a community that values who they are and what they have to offer. We can refer to other services and agencies to support the disabled person to achieve their plan.
