Description
- offer help and support to families/whānau
- facilitate and follow up hospital discharges
- provide short and long-term follow up for children
- liaise with other health professionals to provide the most effective and efficient care available
Our hours of operation are Monday to Sunday 8am - 4.30pm.
Referral Expectations
Waitematā DHB Children’s Community Nurses accepts referrals for Neonates, Children and Young People (up to 16 years) who live in the WDHB catchment area.
Referrals to the Waitematā Children’s Community Nurses are accepted from, but not limited to:
- Newborn and Special Care Baby Units - North Shore Hospital, Waitakere Hospital and Auckland Hospital.
- Inpatient paediatric wards - Rangatira, Starship and KidzFirst.
- Emergency Departments - North Shore Hospital, Starship and Waitakere Hospital
- Community agencies e.g. GPs, Plunket, Taikura Trust, CCS.
- Paediatric Outpatients Clinics.
Please use e- Referrals to refer to the service. Referrals are triaged by the nursing team and actioned.
